This form will be used to calculate your contact hours and deliver your CE certificate for the Ohio Occupational Therapy Association 2026 Annual Conference. Once your submission is received and verified, your certificate will be generated and sent to the email address you provide below.

How to Complete Your Session Selections

Below you will find the conference schedule organized into conference blocks. Each block lists the courses that took place during that time period. Please read each block carefully, as the selection format varies:

Checkboxes: Some blocks contain checkboxes, which allow you to select multiple courses. This is because those blocks include 30-minute, 1-hour, and 2-hour courses that occurred within the same block of time. Select every course you attended in these blocks.
Single Selection: Other blocks use a single selection option (radio button or dropdown). In these blocks, all courses occurred simultaneously and were the same duration, so you may only select one course per block.
