This presentation highlights the co-creation of an occupational therapy intervention among clinical occupational therapists and researchers. The intervention, an occupational therapy manualized, stroke-specific, health management intervention, was collaboratively tailored for implementation in an in-patient skilled nursing facility (SNF) within a large safety-net health system. The key ingredients of the program will be discussed including duration, frequency, and location of the treatment sessions; content included in the manual modules; and delivery methods used by the occupational therapists. Implementation evaluation approaches used to assess feasibility of the stroke health management intervention will be shared, including evaluation of patient needs, rehabilitation practice patterns, and policies and procedures at the SNF. Practical strategies for tailoring interventions will be presented. By the end of the session, attendees will be equipped to describe essential ingredients of a stroke management intervention, identify implementation strategies, and identify evaluation approaches. Discussion will focus on barriers and novel approaches to implementing evidence-based occupational therapy interventions.

Presenter bio:
Dr. Valerie Hill is a tenure-track Assistant Professor in the Division of Occupational Therapy at The Ohio State University College of Medicine, where she directs the Healthy GOALs Life Management Laboratory. She earned her Ph.D. in Health and Rehabilitation Sciences from Indiana University Purdue University-Indianapolis, with a research focus on the relationship between hand sensation and occupational performance after stroke. Her career bridges extensive clinical expertise with a robust research portfolio centered on developing and testing community-based lifestyle management interventions—particularly the Healthy GOALS intervention—to support stroke survivors in managing their health, reducing secondary stroke risk, and enhancing participation in meaningful activities. Dr. Hill’s work has been funded by the National Institute on Minority Health and Health Disparities through a K99/R00 Pathway to Independence Award, and she has published widely on stroke rehabilitation, lifestyle interventions, and implementation science. She is deeply engaged in mentoring students and clinicians, advancing diversity and inclusion initiatives, and fostering community partnerships to translate evidence-based stroke care into real-world settings.
