In this educational session, the fundamental components of a manualized health management intervention and implementation recommendations will be presented. Originally designed for community-based delivery, the Healthy GOALS intervention was redesigned for delivery in an in-patient skilled nursing facility (SNF). The manualized intervention, culturally tailored for stroke survivors, incorporates occupational therapy best practices with evidence-based approaches to stroke management. The manual provides guidance for occupational therapists to address critically important topics that are often overlooked during rehabilitation. Co-created by the presenters, SNF clinical occupational therapists and a stroke rehabilitation researcher, the manual is uniquely tailored to in-patient rehabilitation settings.

The intervention features education, training, and resource sharing for patients with stroke as they prepare for discharge. The key ingredients of the program will be discussed including duration, frequency, and location of the treatment sessions; content included in the manual modules; and delivery methods used by the occupational therapists. The goals and expected outcomes of the intervention will be covered, specifically related to patients acquiring knowledge, gaining skills needed to self-manage their stroke conditions, and living a healthy life upon discharge.

Practical strategies to implement health management interventions during occupational therapy services will be shared, including evaluation of patient needs, rehabilitation practice patterns, and policy and procedure considerations. Time will be allocated for attendees to discuss barriers to implementation and brainstorm tailored approaches to incorporate health management interventions into daily practice.

Presenter bio:

Dr. Valerie Hill is a tenure-track Assistant Professor in the Division of Occupational Therapy at The Ohio State University College of Medicine, where she directs the Healthy GOALs Life Management Laboratory. She earned her Ph.D. in Health and Rehabilitation Sciences from Indiana University Purdue University-Indianapolis, with a research focus on the relationship between hand sensation and occupational performance after stroke. Her career bridges extensive clinical expertise with a robust research portfolio centered on developing and testing community-based lifestyle management nterventions—particularly the Healthy GOALS intervention—to support stroke survivors in managing their health, reducing secondary stroke risk, and enhancing participation in meaningful activities. Dr. Hill’s work has been funded by the National Institute on Minority Health and Health Disparities through a K99/R00 Pathway to Independence Award, and she has published widely on stroke rehabilitation, lifestyle interventions, and implementation science. She is deeply engaged in mentoring students and clinicians, advancing diversity and inclusion initiatives, and fostering community partnerships to translate evidence-based stroke care into real-world settings.
